OFFICE HOURS, APPOINTMENT SCHEDULING, AND APPOINTMENT CHECK-IN
Our clinicians are seen by appointment only. Counseling hours and days vary, depending upon the individual clinician’s schedule and counselee’s needs, but appointments typically occur on weekday mornings, afternoons or evenings. To make an appointment, email [email protected] or call her at 330.650.9533 x220 Monday through Thursday from 9 a.m. – 1 p.m. Confidential messages can be left anytime on Nicole’s voicemail or email, and they will be returned promptly after being received. Follow-up appointments are made either through Nicole or your individual clinician, depending on your clinician’s preference.
SERVICES, FEE SCHEDULE, AND CANCELLATION/NO-SHOW POLICY
Our fee for Licensed Professional Clinical Counselors (LPCC) and Licensed Professional Counselors (LPC) is $55. The average session length is 50 minutes, which includes additional time for the clinician to make notes, complete assessment forms, make telephone calls and referrals when needed, and to plan for future sessions. Every effort will be made to begin and end each session promptly, however, our commitment to respond to priority needs such as psychological emergencies and client safety may require flexibility and understanding on the part of the client.
Payment for each session can be made via cash or check, and checks can be made payable to Christ Community Chapel. It is very important to note that insurance is not an acceptable form of payment. Payment is expected to be made at the time of service and should be given directly to your individual counselor before or after the session. If you are unable to pay the $55 fee due to financial difficulties, please discuss the option of benevolence with Nicole prior to your first session.
If you are unable to attend a counseling session, please make every effort to cancel your appointment 24 hours in advance. We understand that emergencies and unforeseen events do happen.
We require a 24-hour notice should a client need to cancel an appointment. By neglecting to do so, we reserve the right to charge $55 per missed session. Exceptions are made based on emergency situations such as lack of transportation, illness, etc. This fee must be collected before a subsequent appointment can be scheduled. We reserve the right to terminate counseling services due to multiple late cancellations and/or no-shows or failure to make payments for missed sessions. EXPECTATIONS FOR FIRST SESSION
Your first visit is a time for you to begin to know us and how we operate and for us to get to know you and your concerns. During your first session, you can expect the counselor to:
- Inquire about concerns
- Inquire about your background
- Review our policies and procedures
- Answer any questions you may have
On the day of your first counseling session, be sure to bring all forms that your counselor asked you to complete in advance (intake form, disclosure and consent form) which you received via email from Nicole, as well as the $55 payment. It will also be beneficial to the counseling process if you bring with you a list of ideas or concerns you would like to address, goals you would like to pursue and any questions you may have.
STATEMENT OF PRIVACY
Our entire staff will keep confidential anything and everything that you choose to discuss in your session. However, it is essential that you are aware of and understand that there are limits and exceptions to this confidentiality which include:
- Meeting with a professional supervisor, which is required by the CSWMFT Board for all Licensed Professional Counselors, as well as other professional staff during individual and group supervision.
- Your counselor reasonably suspects/determines or client reports intent to harm self or others (Tarasoff Act).
- There is evidence or reasonable suspicion of or client reports abuse against a minor, elder person (65+ years) or individual with a disability.
- On the rare occasion that a court subpoenas client records.
- If/when the client requests in writing that the counselor provide/communicate information regarding the client to another person.
Additionally, confidentiality of information communicated electronically cannot be guaranteed due to the inherent risk of electronic service delivery.
